Can correction of a breast lift be done without disturbing the implants that were put in at the same time?

Q:

I am 54 and had a a breast lift with 170 ml submuscular implants.  After 2 years, though soft, the bottom crease areas have indents and look irregular.  The vertical scar is not smooth.  The nipple scars are outside the areola and white.  At the sides, my skin is lumpy looking.  Can the breast lifting part be repeated to correct these problems without disturbing the implants?  Can the nipples be re-done or the scars pigmented?  I would be happy if I had to go smaller.

A:

You obviously have may concerns after your breast augmentation.  Without actually seeing what you are describing it is difficult to give you an exact answer.  Yes, scars can usually be revised to be less noticeable.  You may be describing folds and wrinkles from the implants that sometimes can occur following breast augmentation.  Please seek evaluation from your original surgeon on your issues. A:

Yes, a lift that is confined to removal of excess skin and scar can be performed without disturbing the implants.  This is all dependent on an examination of your skin and soft tissues in conjunction with your implants.

You seem to have multiple issues with scars and lumpiness.

The lift can be repeated, or scars can be revised.  The lumpiness needs to be evaluated for cause, which means an in person examination.  Is it the implant or the tissue overlying the implant?  What type of implants do you have?

The breast lift needs an evaluation for the proper position of the implant, and may require revision with repositioning or exchange of the implant in a better position that fits the lift revision.

A:

It sounds like what you need is scar revision except for the "lumps."  Scar revision after breast lift-augmentation is frequently successful in correcting what you describe.  It does, however, have to be done with long lasting buried sutures and you will need to support the breasts for awhile.  You need to have the "lumps" evaluated since they could be many things, including breast tissue masses.
