How do I find out how much my insurance covers for an arm lift?

I am 270 pounds and I cannot fit long sleeve tops. I would like to have an arm lift and I believe it is medically necessary. How do I find out how much my insurance covers? 

A:

Brachioplasty, arm lift is a cosmetic procedure. I can not think of any condition to make it medically necessary.

I do believe asking the insurance to pay for a purely cosmetic procedure is fraud
