Q:

Can the placement of silicone implants be controlled?

I recently had a breast augmentation. The implants were placed over the muscle as I had sufficient breast tissue to allow the surgeon to do this. My surgeon advised me that the only way to position the implants was in the actual breast cavity itself. I wanted the high sort of look - probably what you would call a more fake look.  I had the surgery done at the end of July last year.  I feel my breasts look a tad droopy - they are of course fuller - I had 310 cc's placed.  Is there another way to position the implants?  In other words should I seek the advice of another surgeon and have the breasts redone or is what my surgeon said, correct. I'd appreciate your comments.

A:

It sounds as though you had some laxity of your breasts prior to surgery and a modest implant size. The placement of the implant is centered on your nipple and in your case, above the muscle.  If you are trying to achieve a fake look, it sounds as though you might consider either having a larger implant placed or possibly a lift of some kind.  It is difficult to say without examining you.  I would return to your plastic surgeon and discuss your concerns with him/her.

I know after breast augmentation, you should wear loose fitting shirts (button down). For how long should this be done?

A:

The only way this will affect your recovery is to make it easier to get dressed.  Unless your plastic surgeon has told you otherwise, I would say you can progress to your regular clothes as you see fit.

I am 18 years old and have problems with the weight of my breasts. At what age can I consider having breast reduction surgery?

A:

Woman can undergo breast reduction surgery virtually at any age but each case must be individually evaluated by a plastic surgeon.  I have performed breast reduction on women as young as 16 and as old as 70 with good results.  You, as a young woman, need to realize that your breasts will change over time whether it be from weight changes, future pregnancies, etc.  I would recommend that you seek consultation with a Board Certified Plastic Surgeon who can review your medical history, examine you, and discuss the procedure with you to include the anticiapted benefits, alternatives and potential risks and complications.

Breast Reduction

I am a 24 year old female looking for help with my appeal for a breast reduction. I am a 32-DDD and weight about 130 pounds, healthy BMI. I met with a plastic surgeon who explained the process and qualifications to me. My plastic surgeon submitted my claim and it was denied because the amount of tissue removed from my left breast was 150cc less than what my insurance requires; however, my right breast was almost double the required amount for removal. I am currently in physical therapy to help document my neck and back pain. Another reason they denied my claim was that I don't have deep grooves in my shoulders. I need some help - please advise if you think I meet the criteria and have a legitimate claim for appeal.

I cannot speak for your health care insurance but there are often times appeal processes, especially with cases of marked asymmetry.  I would suggest you request an appeal and have any other of your healthcare providers (your physical therapist and primary care physician) write a note as well in favor of your claim.

Breast Reduction

I had a breast lift with areola reduction. After this procedure, I lost sensation in that area. Is there a procedure to correct this?

A:

The nerves that supply sensation to the nipple and areola can be stretched or divided with a breast lift operation although this is certainly something we strive to avoid.  I have had patients who had a decrease in sensation early after surgery who regained sensation weeks to months after the procedure as the nerve repaired itself and the swelling subsided. I would discuss your concerns with your Board Certified Plastic Surgeon.
