Please advise me how to correct a liposuction gone wrong. I have large wrinkles across the tops and inner sides of my arms that were not there before my upper arm reduction procedure. I am a small thin person and have no fat for a transfer. I need them tightened. Can you please advise me of my options?
A:
Without an examination it is difficult to comment on the best solution. Occasionally skin reduction procedures can help. Fat injections can also be of benefit. Depending on the size of the contour problem, adequate stores may be available even in smaller patients to harvest for this problem.

What are the pros/cons of breast reduction Liposuction vs. Surgery?
A:
Liposuction of the breast can down-size the breast only modestly. It works best on fatty breasts in older patients and less well on dense breasts found more commonly in younger patients. It does not reduce loose skin or correct sagging. The incisions are very small and hidden easily.
Breast reduction surgery can reduce breast size to any degree desired. It re-shapes the breast and tightens the skin. The scars are longer and at a minimum circle the areolae and extend from the bottom of the areola to the crease underneath the breast.
Both procedures cause moderate soreness for two to three weeks. Swelling persists for three to four months. Normal activities may be resumed with either procedure in about three weeks.

I had a breast reduction bringing me from a G cup down to a DDD. I have dense heavy breasts and have gained some weight making my breasts large again. What is the risk of having liposuction done? Will the scar tissue interfere from the previous reduction surgery?
A:
Liposuction is difficult to perform when the breast tissue is very dense. It works better in fattier breasts. Significant liposuction of the breasts may also result in sagging skin, as it does not result in significant skin tightening. The best option at this stage may be to try and lose some weight and see if that does not have a positive effect on your breast size. A secondary breast reduction can be done to reduce your breasts further if that does not help.

After having a facelift, what is the likelihood that skin necrosis will cause permanent scarring? The area affected is about 1/2 inch wide and about 1 1/4 inches long, just in front of the ear.
A:
These healing problems do occur from time to time, but facial scars are generally easier to treat than scars in other areas. Follow your surgeon's instructions for wound care until the area is completely healed. The area will be quite pink at that point. Then ask about applying silicone gel sheeting to the area. It is a thin clear or flesh colored sheet with adhesive backing. This treatment helps fade the pigmentation more quickly and keeps the scarred area flat. Usually it is worn twelve hours a day for three to six months. I have seen impressive results. Best of luck.
