Q:

Can I have my 700cc breast implants removed?

I just got a 700cc breast implants on May 17th but I want them out. Since I got them I have only been sick with severe stomach and back pain. Can I get them removed without replacement?

A:

You can have your newly placed breast implants removed, but, at less than two weeks, I would suggest waiting a while longer before making this decision. The symptoms you are having probably have nothing to do with the implants. If anything, you may need them reduced in size to a more reasonable size. Before deciding, see a board certified plastic surgeon and discuss all your worries.

Q:

Will a tummy tuck using redundant tissue fix a large incisional hernia?

I had weight loss surgery (VSG) about 2 years ago and developed a large incisional hernia. My bariatric surgeon said he can fix it without mesh using a tummy tuck and said something about using the redundant tissue developed from the weight loss. I've been trying to find information on this surgery and recurrent rates but can't seem to find anything. If anyone knows this information I'd appreciate it. Thanks!

A:

There are multiple ways to repair incisional hernias during tummy tucks. You have significant excess muscle fascia (the heavy tissue encasing the muscles) because of your weight loss. With an abdominoplasty, this is totally exposed. This can be imbricated and reinforced to repair the hernia. If it appears that you need extra reinforcement, then you could use an artificial mesh, an acellular dermal matrix or a piece of your own dermis taken from the skin that will be discarded. All of these things work well. The problem lies in where your incision is. Some are easier to repair than others and some create significant problems in blood supply to the tissue beyond the incision that can reduce the ability for the tummy tuck incision to heal. Therefore, make sure that whomever does your tummy tuck has significant experience in abdominoplasty. This is where the experience needs to be. Any good plastic surgeon can also repair the hernia.

Q:

Are augmented breasts different to touch?

I am considering breast augmentation surgery but I am concerned with the result. Do they feel same as natural breasts or is it noticeable at the first touch that they are not natural?

A:

Very frequently the augmented breast feels very natural and it is impossible to tell that there is an implant present. I have even had the patient's family doctor or gynecologist unable to tell the difference. This is true even with implants over the muscle when the procedure is done correctly. The more breast tissue you have over the implant, the less likely you are to feel the implant. The most likely place to feel the implant is interiorly if the implants are saline, since saline has a different feel than fat. Silicone implants, even there generally feel like normal breast tissue. Other things make the implants noticeable. Most of these have to do with the choice of implant and the surgical technique. Because of this, choose a board certified plastic surgeon whose results do not look fake.

Q:

I had a tummy tuck and liposuction in 2002, more liposuction in 2010 - why is my stomach still not flat?

I had a tummy tuck and liposuction in 2002.  In 2010, I had more liposuction and my stomach is still not flat.  Why?

A:

Your problem is not uncommon. The most frequent reason that one continues to have a prominent tummy after an abdominoplasty and liposuction is fat around the gut. This is particularly true at your age when your metabolism slows down. This accumulation pushes the skin and superficial fat out from within. Also, the same thing that causes the fat to accumulate around the gut causes the fat to reaccumulate superficially in the remaining fat cells of the abdominal wall. No surgical procedure will correct this. The only thing that will do so is a change in your eating habits. Notice I did not say diet. Diets do not work. You must gradually reduce your taste for sugar and carbs and processed or cooked vegitables.See "You on a Diet" by Drs. Rosen and Oz.

Q:

Which liposuction should I choose?

I am a 33 year old female,  5'5" and weigh 175 pounds.  The only surgery I have ever had is a D & C.  I just quit smoking so that I can be healthy to have liposuction on my upper and lower abdomen. I gained my weight from birth control medication which I stopped taking last year.  I've joined a gym and do boot camp every other day but still cannot lose the weight.  I'm unsure as whether to do laser lipo or normal lipo.  Which one heals faster with less recovery time, less visible scarring and discomfort?

A:

It really does not matter what type of liposuction you have. They all can have the same results and the same rapid recovery. It is the surgeon that makes the difference. Many of the people doing liposuction are not even surgeons and have had almost no training. It is the training and experience that makes the difference in results and recovery. Therefore, make sure your "surgeon" is really such. All members of this society are board certified and surgeons with at least 5-8 years training before starting their practice. Also ask about experience in liposuction and results.
