Q:

Should I be concerned about a trans-areola augmentation affecting breastfeeding?

I had my breast implants with a trans-areola incision which I have regretted all my life.  I am now pregnant and extremely worried about complications with breast feeding.  I want to know is it just a matter of being unable to breast feed or are there other issues like milk rotting in the glands because it cant flow?  What can I do to avoid risks?

A:

The ability to breast feed after surgery is difficult to study but I have women who breast fed after this procedure. A periareolar incision will only pass through about 30-40% of the milk ducts leaving 60-70% intact. The unaltered breast will function normally while the obstructed ducts will not drain and the fluid will be absorbed. I am unaware of any risks other than a compromised ability to breast feed. Many woman who have not had surgery have difficulty breast feeding. Do not worry. There are lactation consultants who will visit you in the hospital to help you with the process. This is a time of joy and you should be looking forward to the experience. For more peace of mind you can talk with a board certified plastic surgeon in your community. Good luck.

Q:

Can I have my belly fat injected into my breasts?

I'm a 34B cup and I want to go up one cup more.  I don't even fill the present cup the way I want to.  I'm trying to look as natural as possible.  I have some fat in the abdomen.  Can I have my own abdominal fat injected into my breasts?

A:

Yes, in fact you sound like the ideal candidate for this procedure. I say this because the fat grafting technique is useful for small to modest augmentations. Abdominal fat is an excellent donor site and you will have the cosmetic benefits of contouring the abdomen while enhancing the size of your breasts. See a board certified plastic surgeon to evaluate you and help you make the decision which is right for you.

Q:

What can be done for bulges from silicone bags folding over after breast augmentation?

Is using Belladerm the only choice to fixing bulges after breast augmentation surgery?  My doctor wants to charge me for another surgery to fix the bulging problem.

A:

Answering this question accurately would require a formal evaluation to determine the exact cause of the bulging. If the implants are above the muscle converting them to below the muscle will resolve some of the bulges in the upper pole of the breast. Fat transfer to the breast can minimize the appearance of bulges if the upper pole of the breast is thin. The dermal tissue you mentioned is another good option for repairing bulges when the tissue is thin and support is needed.

Seek out a board certified plastic surgeon to help you better understand your options. I hope this was helpful.

I am a 36-year old mother of two. I am planning on having a tummy tuck within the next two months. I have lost about 20 pounds and need to lose another 10 pounds. I was told by an associate that you normally lose additional weight after the tummy tuck surgery and I should not worry about the extra 10 pounds. Is it normal for a person to lose a little weight after the surgery and should I have the procedure without losing the additional 10 pounds?

A:

Ideally body contouring surgery should be done when you are at your goal weight. This is more critical in people who are 20 pounds or more overweight. You will lose weight during the surgery with the removal of tissue and most people lose weight during their recovery but you don't want to count on this contributing to your final contour. Try to lose it but if you can't, I would not be concerned about the 10 pounds. Seek out a board certified plastic surgeon to evaluate you and comment on this.

All the best.

How long should swelling and numbness last after a face and neck lift?

A:

Swelling after facelift is going to vary based on the technique your surgeon uses. Generally speaking swelling which is obvious to the general public resolves by 10-14 days. Numbness will be present anywhere the skin has been raised. Sensory nerves are very forgiving but take time to heal completely. Sensation will gradually return over weeks to months. Seek out a board certified plastic surgeon to go over their technique and the specific recovery. I hope this was helpful.
