Q:

Would it be a risk and unsafe to have a tummy tuck 11 years after spinal fusion?

I would like to have a tummy tuck.  I had a spinal fusion 11 years ago which left me with a 12 inch scar up the left side of my chest.  At that time, the surgeon also removed a rib for a graft.   I saw a plastic surgeon recently to discuss my having a tummy tuck/mommy makeover.  He expressed concern over the scar and the risks performing the surgery because of the old scar and blood flow.  However, another surgeon said it would not be an issue.  Who is right???

A:

Tummy Tuck After Spinal Fusion:

This needs to be prudently evaluated by both your Spine Surgeon and your Plastic Surgeon. If your spine fusion is stable and does not require significant long term flexation with your abdominoplasty and with approval of your spine surgeon, one can undergo an abdominoplasty procedure.

Q:

Is it medically necessary to remove a ruptured silicone breast implant inserted in 1980?

I just found out that a silicone breast implant that was inserted in 1980 has ruptured.  Is it medically necessary to remove the ruptured implant now?

A:

Ruptured Breast Implants:

If an implant has ruptured and been documented by an MRI, I would highly recommend that it be removed to prevent dissemination silicone formation of granulomas in the breast area.

Q:

After losing 100 lbs. through bariatric surgery, will a tummy tuck make my stomach flat if I still have fat in the stomach area?

I had Bariatric surgery in 2000 and lost over 100 pounds.  I presently weigh 175 pounds and am 5 feet tall. My stomach is still very large and seems full of fat, (not just loose skin to pull).  Without losing any more weight, will a tummy tuck make my stomach flat?

A:

Tummy Tuck Alone After Major Weight Loss:

In general, if you have lost more than 100 pounds, then a traditional abdominoplasty may not be enough. You may need to have a central bodylift, but this depends on how much skin laxity you have in the flank and back areas. This would need to be examined by a Board Certified Plastic Surgeon with expertise in Bariatric Surgery and management of massive weight loss.

Q:

I want implants that were inserted one month ago removed - will a capsulectomy be needed?

I am 27 years old and my breast size was an A cup.  I had 300 cc's, smooth, Silicone gel, high profile implants, under the muscle put in 5 weeks ago.  I really want them removed.  Will I need a capsulectomy?  Will my breasts be the same size as before or even smaller?  Thanks

A:

Early Implant Removal and Need for Capsulectomy:

If you had your implants placed in 5 weeks ago and want to have them removed, there is no need for a capsulectomy. There should not be a capsule at all and this would add morbidity to the procedure.

Q:

With facial issues needing work done and skin tightening, will I have a natural look after the surgery?

My face seems to have fallen into deep lines on both sides of my mouth creating a sag.  I have a "gaddle"under my chin.  There seems to be a muscle that runs from the center of my lower front teeth down to my voice box (I have had this all of my life).  It seems like the top of my face does not match the bottom.  Can these issues be fixed without having that horrid obvious "done" look?  In other words, will I have a natural look after the surgery?

A:

Natural Look in Facelifts:

The key element in having a natural look in facelifts is to perform the procedure correctly the first time and that is based on pre-operative analysis and the facelift to do of the deep layers of the facial fat compartments as it has been described.
