Can I have a third blepharoplasty done at the age of 79?
I had my first blepharoplasty done at age 45, and the second one three years go. My eyelids are now sagging again. Is it advisable to have a third procedure done now at age 79?
A:
Tertiary Blepharoplasty at Age 79 – One has to be very cautious at this age doing a blepharoplasty, especially because everyone has dry eyes at this age. If you are going to do a blepharoplasty, one needs to be very conservative to preserve globe function and to not exacerbate dry eyes.

Should I be concerned about a trans-areola augmentation affecting breastfeeding?
I had my breast implants with a trans-areola incision which I have regretted all my life. I am now pregnant and extremely worried about complications with breast feeding. I want to know is it just a matter of being unable to breast feed or are there other issues like milk rotting in the glands because it cant flow? What can I do to avoid risks?

Trans-areola Breast Augmentation and Breastfeeding – Trans-areola breast augmentation done in the past should not really affect your ability to breastfeed as this does not usually transect the areola ducts. However, my preference is to not perform a trans-areola breast augmentation, but rather a short, well-hidden inframammary approach for this reason. Also, this is a more visible scar.

Can I have my belly fat injected into my breasts?
I'm a 34B cup and I want to go up one cup more. I don't even fill the present cup the way I want to. I'm trying to look as natural as possible. I have some fat in the abdomen. Can I have my own abdominal fat injected into my breasts?

Fat Injections in the Breast
In selected cases, one can either supplement breast augmentation or breast asymmetry with addition of fat for shaping and contouring in addition to implants. As a stand alone procedure, one cannot usually get more than one size change with fat augmentation. This can be done safely in the hands of someone with expertise and experience in doing fat augmentation.

Will rhinoplasty for a deviated septum alter my singing voice?
I have a deviated septum and wish to have it fixed with rhinoplasty. In addition, I also wish to slightly extend my nose. (to make it sharper) because it is very small. I sing professionally. Would it be all right to reshape my nose as well in addition to just fixing my deviated septum? When I breathe with my nose and sing, my pitch is limited and my voice becomes nasal at higher pitches; when I breathe through my mouth, I can hit higher notes and the nasal tone is not present.
A:
Deviated Septum & A Singing Voice – Obviously, it is very important to understand that you may have some changes in your nasality and even your singing voice, potentially, although it may be small if you correct the deviated septum. I would consult with your speech expert in consultation with your Board Certified Plastic Surgeon who will perform your rhinoplasty to make sure the changes are minimal, if at all.

What is the best age for a woman to have a facelift?
A:
Optimal Age For a Facelift – The optimal age is not chronological, but rather genetic. Aging depends on whether one has genetic aging by fat atrophy and/or jowling, both are major components of facial aging. In general, when a patient has these components, if it is volumetric deflation then one can use fat augmentation, but with jowling then one needs to have skin or deep layer tightening, a “Lift & Fill” facelift. A “Lift & Fill” facelift is “lifting” the central lateral portion and “filling” the deflated central fat compartments. In general, this is usually in the late 40’s/early 50’s. It is best to do your facelift earlier rather than later for longevity.
