Where is the best area in my breasts for the doctor to cut open for my implants?
Actually I would like implants but I am wondering where should the doctor cuts so nobody can see, around my nipple or under my breast?
A:
Best Incision for Breast Implants -
The optimal implant incision is determined both by the patient and the surgeon. My preference is to do a small inframammary fold incision which is imperceptible within weeks to months and has met lower revision rates in my personal experience of over 20 years and more than 2,000 breast augmentation patients.

Is it possible to reconstruct a bone graft done in the nose ?
I have a cleft lip/palate which also affected my nose. My nose on one side is deflated and the other side is larger.
I have a bone graft that builds up my nose so it is not flat. It goes to the tip of the nose. It causes me discomfort and the tip of the bone is apparent thru my skin. I want to have my nose reconstructed and the bone graft taken out. Is it doable to put fillers in the tip to make it look like a nose and to reconstruct the deflated side to match the other side so it will be more symmetrical?
A:
Management of Cleft Lip & Nasal Deformity – Nasal reconstruction in a cleft lip is very difficult and is a complicated operative procedure. It may require rib cartilage rather than bone in most instances. As in secondary repair or deformity I usually use ear or rib cartilage, however, the deformity correction depends on the type of problem you have. Most of the time, there are deficiencies on the cleft alar base side and you can fill the alar base and the periapical area with Radiesse to produce a better nasal base platform.

Why do I have *recurring* bruises and ridges months after a Restylane injection?
Three months ago, I had only 0.1 ml Restylane injected under my eyes. Two weeks later, skin under my eyes started to flake; ridges of skin & deep red bruises in my tear troughs appeared which took a week to go away. Bruises and ridges came back 1 1/2 months post-injection, and again now, 3 months post-injection. Why does this keep happening? Can the filler be moving from time to time and blocking a blood vessel? Is it a permanent damage? What can be done to prevent future episodes?

Bruising After Restylane Injections – It is not uncommon to have slight bruising around the tear trough areas after Restylane or any hyaluronic acid filler. One can avoid or minimize this by using Lidocaine with Epinephrine and mixing it in with the Restylane, as well as also injecting very small amounts very gently with minimal massage. If you do get bruising, apply ice immediately.
Q:
While doing a tummy tuck, can an incisional hernia be repaired at the same time?
Previously I was surgically cut from just under my breast to my groin and have now developed a hernia there. Is it possible to have a tummy tuck at the same time the hernia is repaired? I also think I have a lot of scar tissue in the area. Would this surgery be more risky and what are the chances of a good result?

Incisional Hernia Repair and Tummy Tuck – It is quite common to repair an incisional hernia at the same time as a tummy tuck. However, if it is a large hernia or recurrent hernia, I usually involve your general surgeon as well.

Botox for frown lines and Bells Palsy/facial paralysis history
Is Botox safe to be used to smooth frown lines if I had Bells Palsy once and facial paralysis on one side as well? How about Radiesse on my cheekbone area as well?
A:
Botox & Bells Palsy – To use Botox to provide symmetry on both the normal and the effected side the Botox injection must be based upon a careful analysis of what has caused the asymmetry. Radiesse is also fine to place on the cheekbone for facial shaping and symmetry as well.
