Q:
Am I too heavy for breast reduction to be an option?
I am 21, 5'5", 190 lbs with a bra size of 38F. I suffer from back & shoulder pain, as well as the problem of finding clothes that don't make me look like a tramp. Guys think I'm easy cause of my chest size while I can't imagine letting someone see my disfigured upper body.
A:
No - your weight is not an issue, if you are have significant issues with your back and shoulders, and you have seen a chiropractor/ back specialist regarding this and it is their opinion that your large breast size is contributing to your issues, then your insurance may cover the surgery. This varies from one insurance company to the next. Often the large breast size prevents you from exercising and losing weight. Having the surgery in a person with your symptoms often can be life-changing. I would suggest seeing a surgeon who is board certified and is well versed in this type of surgery.

I want to know if there are different cuts for a tummy tuck that can be done, such as on the left and right side of the belly button. Also, can a tattoo cover the scarring from this?
A:
The usual incision for the tummy tuck should be curved and should be as low on the abdomen as possible. The scar around the belly button should either be oval or triangular in shape. I do not place incisions on the side of the belly button. With regard to tattooing over the scars - I have had many patients that have done this either to the belly button scar or to the lower abdomen scar, or both and the scar as a result becomes almost invisible. I would not suggest tattooing over your scar until you have cleared this with your plastic surgeon.

I recently had nose reshaping done on 12/19/2009. I was unaware that silicone was put in the tip of my nose and I would like to have it taken out. My nose is still swollen and numb. How soon will I be able to have the corrective surgery done?
A:
I am a proponent of not using artificial materials in the nose; however, many surgeons do use synthetic materials without problems, and with good success. I also believe that if you prefer to take the silicone out, that this should be done sooner rather than later. I don't believe that there is any point in waiting. In any event, most of the edema/swelling should have resolved by now. I would suggest re-exploring the nose, removing the silicone, and possibly replacing it with your own cartilage if necessary. This can be obtained either from your nasal septum, or from the ear.

I am looking into breast augmentation but I want more information on areola reduction surgery. Are all ASAPS Board Certified surgeons capable of performing this procedure? Can you recommend a cosmetic surgeon in South Carolina for my procedure? Thank you!
A:
Nipple-areola reduction can be done at the same time as a breast augmentation. The implant can be put in via a peri-areolar incision, thus avoiding additional scarring. Most ASAPS member surgeons, because of their focus on aesthetic surgery, are able to do this type of surgery. The scar that would result from this surgery would be a scar around the areola. There would initially be some puckering of the skin around the areola, but this would diminish with time. Additionally, the breast may appear less "conical" or more flattened in its shape after the surgery, and this may be compensated for by placing a higher profile implant.
