Instructions

Please enter your user name in lower case letters with no punctuation or spaces as follows: first initial, middle initial (if you have one) and full last name. Do not include any abbreviations for Jr. or Sr. Example: Robert William Smith, Jr., MD = rwsmith

For your default password (you can choose a different password once you are a registered user), use your 6-digit birth date and the last four digits of your ASAPS Member ID number. If you have a three-digit ASAPS Member ID number, enter 0 before your ID number. Example: A birth date of February 1, 1965 and a Member ID of 987 = 0201650987

Click on sign in to complete the process.
